The Momentum Program provides intensive and innovative case management services to individuals requiring intensive care in order to remain in the community, including individuals transitioning out of the Mental Health Institutes, children and youth with significant psychiatric and/or multiple system involvement, and adults with behavioral health needs and significant barriers to transition.
