[CPN] Community Services Block Grant, (CSBG) Survey

Hi Friends,

 

We need your assistance in distributing the Community Needs Assessment Resident Survey to low-income residents that live in your neighborhood. The information gathered will be used for the primary purpose of informing the Community Services Block Grant committee about how to improve services for low-income people and families.

 

Please distribute this survey to at least five (5) of your low-income clients or encourage them to take the survey online.  This is a confidential survey, so please do not influence your clients opinions.  Please submit completed surveys by August 17, 2012:
