Patient Navigator – Colorado Access Grant

General Purpose: In order to assure optimal clinical outcomes for patients, the patient navigator will work with partner organizations (Community Health Workers) to facilitate access to care and treatment adherence for MCPN patients in Aurora who are at risk for cardiovascular disease (CVD).   This position reports to the Community Services Department.

 

 

Essential Duties/Responsibilities:

  • Coaches patients and reinforces self-management goals regarding cardiovascular disease risk factors, both in the clinic setting and telephonically.
  • Engages patients and their families to participate in the program to reduce risk for cardiovascular disease.
  • Bridges the gap between clinician and patient by assessing needs and obstacles, following up with patients, and addressing health literacy, cultural issues, and social-class barriers.
  • Encourages adoption of habits that are conducive to a higher quality of life.
  • Arranges language interpretation services when necessary.
  • Participates in partnerships with community-based organizations.
  • Obtains and/or develops health education materials for patient, caregivers, and family members as needed.
  • Participates in developing new culturally competent approaches to health education delivery with Health Education team.
  • Provides timely documentation of goals and care plan, and required reporting for all health coaching activities.
  • Collects and enters data related to required process and outcome measures.
  • Continually assesses program effectiveness and adjusts approaches accordingly.
  • Assists administrative staff with preparing required program reports.
  • Performs duties in accordance with established clinic policies and procedures, objectives, quality improvement activities, safety, and environmental and infection control standards.

 

 

Non-Essential Duties/Responsibilities

  • Attends all staff meetings and all other meetings as assigned.

 

Job Qualifications

 

Knowledge, skill and ability:

  • Ability to interact positively and build rapport with patients, coworkers and/or external contacts.
  • Ability to demonstrate the capability to effectively and sensitively respond to the needs and concerns of the full range of MCPN’s diverse patient population.
  • Ability to handle sensitive information ethically and responsibly.
  • Ability to protect the confidentiality of patient, employee and business information.
  • A general working knowledge in the areas of nutrition, physical activity, and overall health and wellness within a community setting. 
  • Strong interpersonal and problem solving skills 
  • Must be able to work independently with minimal supervision.

 

Education or Formal Training:

  • High School diploma or equivalent (GED) required
  • Additional education a plus.
  • Two years in a health care setting preferred.

 

Experience:

  • Bi-lingual English and Spanish required
  • Bi-cultural required; Bi-cultural Spanish preferred.
  • Experience with diverse communities preferred.
  • A general working knowledge in the areas of patient care, health, and wellness preferred.

Working environment/physical activities: Office environment within a clinical setting.  Enters data into computer programs via computer, mouse and keyboard.  Moves about the office environment and occasionally to other locations.  Moves/transports objects up to 25 lbs occasionally. Communicates information to others.  Discerns/analyzes information from others to assist in decision making.
